XSI Mod Tool

Is it possible to create models with this tool, or are they of too many polygons/impossible to export?

Well it's aimed to create models for games so I think it's easy to create lowpoly models. According to the site it can export to DXF and XSI which can then be imported to MilkShape.
Pain is only psychological.

It all depends how much polies you add to the mesh. XSI Mod Tool was created or modified for the development of Half-life 2 (Source Engine), if I recall right. It's quite similar to the older half-life 1 (Gold Source Engine) in many regards, so I'm pretty sure it will work.
